The goal of the Health Home program is to coordinate health and social services that might be needed for an individual or family. This could result in fewer trips to the emergency room or less time spent in the hospital. It also facilitates regular care-from physicians and or other providers.  We offer other auxiliary services, such as, finding housing services, additional education resources and nutritional support.  We assist in making appropriate referrals for essential services, and monitor the overall process and service delivery.
